The Hamilton Communities Health Care Center provides a vast array of medical and nursing services. Our professional staff works closely with the Medical Director to develop a personal health care plan and provide you comprehensive support.

The goals outlined in the personal health care plan include physical and social nurturing and rehabilitation therapy. Once your plan of care is defined, the actual care and service is implemented by  caring nurses and certified nursing assistants and other medical support around the clock. Social Services offers counseling with you and your family members. Dietary considerations are reflected in well prepared and flavorful meals, and resident interaction is achieved through involvement in personal hobbies and educational pursuits.

Hamilton Communities floor plan is intimate and accessible, spacious and sunny. Accommodations are large enough for you to add the personal touches of your own furnishings. All rooms have centrally located emergency call systems, so help is always within reach.

The grounds feature landscaped gardens which have both aesthetic and therapeutic value, providing a backdrop for socialization and interaction through interesting surroundings.

All of our Health Care units as well as our Assisted Living facility are equipped with key pad security systems that offers peace of mind to both family, friends and residents of Hamilton.
